More Than A Home Manager(s) will review placement request and conduct a thorough assessment on prospective resident to determine whether the residents needs can be met with the services identified in the program statement. Residents are required to complete a pre-placement visit, which will include a short program overview, a tour of the home, and an optional dinner with the current residents. More Than A Home services promote clients health, well-being, self-esteem and overall quality of life.
The facility provides each resident with a detailed plan of care within 30 days of admission that details services provided, goals, and level of care needed. MTAH provides transparency of services, by providing regular updates of care, and progress to all stake holders. Our goal is to meet the needs of each resident and to adapt a plan of action as needed.
